Referral → verify Medicaid/PASSPORT → DON assessment & care plan → set up EVV (register caregiver in Sandata) → caregiver clocks in/out each visit → those hours become a claim → submit to payer → payer reimburses → pay the caregiver. You pay the aide an hourly cost; PASSPORT reimburses at its own hourly rate; the difference, minus overhead, is your margin.
